Results from the project suggest that the gel could be used to improve healing treatments and techniques for other complex wound types. Aloe vera is a plant belonging to the cactus family. It contains a substance that is rich in both vitamins and minerals. In contrast, the unique NUS invention takes a comprehensive ‘all-in-one’ approach to wound healing, accelerating the process on several fronts. Results from lad tests of the gel showed that, when coupled with magnetic stimulation, it healed diabetic wounds approximately 240 per cent faster by increasing the cells’ growth rate and more than doubles the amount of collagen produced, which is vital in the healing process.

RESULTS:

Anti-inflammatory hydrogels enhance the recruitment of macrophages to the wound bed and reduce ROS levels. These functions contribute to wound healing by allowing for the transition from the inflammatory to the proliferation stage, increasing the rate of healing, and minimizing the length of the healing period. Some hydrogels show intrinsic anti-inflammatory properties. For instance, CS and its derivatives can regulate the inflammation process by enhancing the secretion of TGF-β, PDGF, and IL-1, which helps accelerate angiogenesis, fibroblast proliferation, and collagen synthesis [ 92, 99]. HA, which is a major component of the skin ECM, also shows inherent anti-inflammatory properties [ 100, 101]. HA is involved in the signaling cascades that trigger the recruitment of inflammatory cells around the wound, enhance cytokine expression, and decrease inflammation to protect the tissue damage [ 102, 103]. Anti-inflammatory properties can also be added to hydrogels by chemically attaching anti-inflammatory agents, such as phenolic compounds [ 78, 93, 94, 96, 98]. Those compounds can be obtained from plant extracts, honey, or antimicrobial peptides. Anti-inflammatory compounds are mostly recognized by the immune modulatory pathways to regulate inflammation and reduce ROS levels around the wound. Stimulating the body’s innate immune system by modifying the hydrogels with targeting agents can also be used to improve anti-inflammatory properties. For example, targeting the sphingosine-1-phosphate receptor could be an approach to activate inflammatory cells [ 104], which helps the transition from the inflammatory phase to the proliferative phase of wound healing [ 105, 106, 107, 108, 109, 110]. Hydrogels containing bioceramics can also exhibit anti-inflammatory activity in wound healing applications [ 111]. For instance, the controlled release of silicon (Si) ions from bioceramic particles that were encapsulated in gelatin/PCL nanofibers reduced inflammation while increasing angiogenesis, collagen deposition, and re-epithelization [ 112, 113, 114]. Brine shrimp lethality bioassay method was used to evaluate the in vitro cytotoxic effect of the formulated AM, AV and AM+AV gels (Fig. ​ (Fig.2a6). 2a6). A concentration of 0.25mg/ml did not affect the viability. At a concentration of 0.5mg/ml and 0.75mg/ml, the mortality rate of the nauplii was 1%. Increasing the concentration (2mg/ml) of gels led to an increase in mortality rate to 3–4% (Fig. ​ (Fig.2 2a6). Beside active ingredients, the applicability of burn wound healing gel is dependent on the properties such as pH, appearance, homogeneity, and viscosity. Carbopol is a widely used gelling agent for producing burn-healing gels for skin application. CMC-Na salt is another agent used alongside with carbopol [ 46]. However, in this study the gel was prepared from human amniotic membrane extract (AM), Aloe vera extracts (AV), and a combination of both AM+AV using 6% CMC-Na salt as a gelling agent. Extensive studies confirm the effectiveness and safety of lyophilized amnion as a wound dressing and grafting materials for promoting the healing process and preventing infections [ 47]. Khorasani et al. , (2009) reported that AV cream or gel could be more effective than silver sulfadiazine cream in treating burn wound healing [ 30]. The end products (gels) in all three conditions evaluated in this study were homogenous, granule less and of a whitish creamy color (Fig. ​ (Fig.1a10, 1a10, 1b3, 1c). In addition, pH of all formulations ranged between 6.5–6.9, and this pH does not interfere with skin physiology [ 48]. Erythema and edema are the common symptoms of skin irritation which lasts for three to 7 days [ 49]. Our formulated gels did not induce any irritation including no erythema or edema on rat skin upon topical application for a period of 7days (Fig. ​ (Fig.4c). 4c). Thus, CMC-Na salt, AM and AV extract formulated gel could be useful as wound healing gel as the gels have good spreadability and consistency. In vitro and in vivo studies clearly and collectively demonstrated the potentials of the formulated gels from amnion when combined with Aloe vera. In this preliminary research, we found that the formulated gels can induce and accelerate the proliferation and attachment of HaCaT and HFF1 cells (Fig. ​ (Fig.3), 3), and promote wound healing in vitro (Fig. ​ (Fig.4a, 4a, b) [ 50]. It showed that the curcumin present in the spice stimulated the production of the growth factors involved in the healing process. It also showed that curcumin accelerated the management of wound restoration.
